COMMUNITY FEEDBACK
Sydney West Area Health Service is committed to the active involvement of consumers, carers and the community in the planning, development, review and evaluation of its services. We encourage consumers, carers and the community to send us comments, provide feedback and raise any issues about the health services.
There are a number of different ways that you can become involved and have some input into the way health services are developed, changed and improved. These include:
• Completing surveys or agreeing to be interviewed by staff who are asking for feedback
• Talking to the staff or service managers
• Coming to public meetings or forums and having a say
• Reviewing policy or documents via this web site
• Becoming a community representative on a committee
• Being active in the care planning of your own health care or the health care of someone you care for
• Sending us an email with your comments
SWAHS ensures that feedback provided by consumers, carers and community representatives will not compromise current or future health services to the individuals who provide the comments
