The Integrated Health Care Cluster brings together Networks and Services whose services are provided solely or to a significant extent “in the community”, or in settings that are alternatives to inpatient care. Services are provided to patients/clients/consumers in community health centres, in their homes, in clinics located in, or adjacent to, hospitals, in nursing homes and hostels, in boarding houses and in other settings such as schools. The Mental Health Network, and to a lesser extent Drug & Alcohol Services, also provide services in non-acute inpatient and/or acute inpatient settings.

Services are provided across all ages and across the continuum of care from health promotion and prevention, through to early intervention, treatment, rehabilitation and maintenance.

 

Drug and Alcohol Service

The Drug and Alcohol Service is a specialist service established to provide assessment and treatment of patients requiring care for substance abuse related problems. The Service provides a range of specialist medical, nursing and psychology services. The Service has developed a high level of capacity to provide prevention and early intervention strategies both within existing service structures and in partnership with other agencies.

The Drug and Alcohol Service has established, in partnership and collaboration with medical services, mental health services, GPs, Community Health Services and NGOs, the capacity to manage complex presentations through the integration of care.

The range of services provided directly by the Drug and Alcohol Service encompasses intake and assessment, medical outpatient services, inpatient and outpatient detoxification, opioid treatment services, specialist hospital consultation and liaison services, specialist drug use in pregnancy service, counselling, psychology, forensic services, prevention, early intervention and advice/support services. There are two designated State-wide services – the Correspondence Program based as Fleet Street D&A Service, North Parramatta and the 15 bed inpatient detoxification unit at Nepean Hospital.

 

Mental Health Network

The Mental Health Network provides a range of services across the continuum of care: mental health promotion and disease prevention, early intervention, assessment, treatment, rehabilitation and maintenance. These services are provided in a variety of settings including the community, acute and rehabilitation inpatient units; emergency departments (eg Psychiatric Emergency Care Centre at Nepean Hospital); community rehabilitation and supported accommodation.

The Network provides community mental health and general acute inpatient services for local residents. Adult intensive management inpatient services are provided for the residents of Greater Western Sydney. Acute adolescent and child and family; forensic; and non-acute inpatient services are provided on a Statewide basis.

 

Primary Care & Community Health Network

The functions of the Primary Care & Community Health Network are: health promotion and prevention; opportunistic and planned, early detection and intervention of health problems and risks to improve and maintain health; assessment and referral of people presenting with selected conditions and issues; treatment of episodic, short term common health conditions; post acute care; ongoing care of chronic and complex conditions in collaboration with more specialised services; provision of extended care in the community; provision of in-reach services to inpatient facilities; whole of government human service programs developed and coordinated by inter-department bodies; and capacity building within a targeted community.

To ensure maximum service accessibility, facilities and centres of activity are located in population centres as close as possible to peoples homes, schools, shopping and other access points such as public transport routes. The design of facilities is “friendly” to young people and those from other cultures. Facilities are also open during the evening for use by health related community groups.

Primary Care & Community Health services are provided by a variety of treatment models in Community Health facilities, homes, clinics, neighbourhood centres, schools and shopping centres. Many services are provided in partnership with others.
